Personalizing Care Management
What is personalized care management? We are glad you asked! Personalized care management is truly client based. We have one focus: OUR CLIENTS. Working closely with our clients, their families and core support circles, we learn what their needs are: socially, medically, physically, what their interests are, what they like to do. Those components, with approval and in partnership with their families, are then utilized to create a care management approach uniquely specific to our client.
Patient care coordinators are available in insurance groups, home health agencies, hospitals, assisted living centers, nursing home and senior living communities. The difference is that our focus is only on our client’s needs and wishes, not facility policies or a supervising board of directors.
